About the Position:

The objective of the Multi-Center MDS Coordinator is to provide support to the MDS and Care Plan Teams in multiple Missouri nursing centers.  This full-time position is a perfect opportunity for the experienced MDS Coordinator who thrives on challenge.

 Position Summary:

The Multi-Center MDS Coordinator will be responsible for coordinating the Resident Assessment Instrument (RAI) process including completion and submission of an accurate Minimum Data Set (MDS) and development and delivery of the interdisciplinary person-centered Plan of Care in accordance with the CMS RAI Manual and all applicable regulations. The Multi-Center MDS Coordinator will establish and maintain positive relationships with nursing center staff, residents and resident representatives, working in collaboration with the Interdisciplinary Team to ensure accurate and timely completion of the MDS in order to maintain regulatory compliance.

Experience and Licensure Requirements:

  • RN or LPN with a current, active Missouri license.
  • MDS experience of greater than two years preferred.
  • AANAC RAC-CT preferred,  but not required.
